Facial expressions are part of how you connect with other people. You raise your eyebrows when you’re surprised, squint when you laugh, and draw your brows together when you’re concentrating. Over time, however, those repeated movements can leave lines that remain visible even after your face relaxes. You may begin to look worried, tired, or stern when you feel none of those things.

Xeomin is a prescription injectable treatment that temporarily softens upper facial lines by reducing the activity of selected expression muscles. It is FDA-approved for the temporary improvement of moderate to severe frown lines between the eyebrows, horizontal forehead lines, and crow’s feet in adults. Because placement and dosing can be adjusted to the individual face, treatment can be designed to soften distracting lines while preserving the expressions that still feel like you.

At Women’s LifeCenter Aesthetic Surgery and Medispa, Xeomin treatment is guided by Elysa Fisher, MD, FACS, a Northwestern-trained plastic surgeon who has been board-certified in plastic surgery since 1992. Dr. Fisher examines how your brows, forehead, eyelids, and surrounding facial muscles work together before recommending treatment. Her goal is not to remove every line or create a motionless forehead. It is to help your face look smoother, more rested, and naturally balanced.

When Your Expression Lines Tell the Wrong Story

Some lines become part of a face in a way that feels warm and familiar. Others begin to communicate something you do not intend. Vertical lines between the eyebrows can make you look frustrated, while horizontal forehead lines may make you appear constantly concerned. Crow’s feet can deepen every time you smile or squint, eventually remaining visible even when your face is still.

Many clients who ask about Xeomin do not want to look dramatically younger. They want coworkers to stop asking whether they are tired, or they want photographs to reflect how energetic they actually feel. Others notice that makeup settles into forehead lines or that a furrow between the brows has become visible throughout the day. These are subtle concerns, but they can affect how comfortable someone feels in conversation, photographs, video calls, and everyday life.

The right treatment is not always the one that eliminates the most movement. Facial muscles support expression, eyebrow position, and balance around the eyes. Relaxing one area too strongly can influence another, particularly when someone naturally uses the forehead muscles to help lift the brows. Dr. Fisher evaluates those relationships before deciding where Xeomin should be placed and how much relaxation is appropriate.

What Is Xeomin?

Xeomin is the brand name for incobotulinumtoxinA, a form of botulinum toxin type A. It is injected into selected muscles and temporarily reduces the nerve signals that cause those muscles to contract. As the targeted muscle activity decreases, the skin above the muscle folds less forcefully, which can soften the appearance of movement-related lines. The effect develops gradually rather than filling the wrinkle with added volume.

Xeomin is currently approved for three major areas of the upper face. These include glabellar lines, commonly called frown lines or the “11s,” horizontal forehead lines, and lateral canthal lines, commonly called crow’s feet. The official prescribing information identifies separate muscle groups and dosing guidance for each area because every line is created by a different pattern of movement.

The treatment is temporary. According to the manufacturer, results commonly last approximately three to four months, although the exact duration varies among individuals. Muscle strength, metabolism, treatment history, placement, dose, and the way your face naturally moves can all affect how quickly motion returns.

What Makes Xeomin Different?

Xeomin is sometimes described as a highly purified or double-filtered neurotoxin. Its manufacturing process removes complexing proteins while retaining the active botulinum toxin type A. The finished prescription product also contains human albumin and sucrose, so calling it completely additive-free would not be medically accurate.

This formulation is one reason some clients and providers consider Xeomin among their injectable options. However, it does not mean Xeomin is automatically the best treatment for every person. Facial anatomy, past response, treatment goals, muscle strength, and medical history remain more important than choosing a product based on a single marketing phrase.

Xeomin units also cannot be directly compared or converted to the units used by other botulinum toxin products. A certain number of units from one brand does not necessarily create the same result as that number from another. Dr. Fisher considers the actual product, treatment area, and individual anatomy rather than applying a formula borrowed from a previous injectable.

What Can Xeomin Treat?

Xeomin is designed to improve dynamic expression lines. These are lines created or deepened by repeated muscle movement, such as frowning, squinting, smiling, or raising the eyebrows. When the muscle relaxes, the overlying skin is no longer folded as strongly, allowing the area to appear smoother.

Frown Lines Between the Eyebrows

Glabellar lines develop between the eyebrows when the corrugator and procerus muscles pull the brows inward and downward. These lines may begin as temporary creases that appear only when you frown, but they can eventually remain visible at rest. Xeomin can temporarily reduce the muscle activity responsible for moderate to severe glabellar lines in adults.

Treating this area requires careful attention to brow position and the muscles surrounding the eyes. The goal may be to soften an intense or tired appearance without creating an unnatural distance between the brows. Dr. Fisher watches how strongly each side contracts because many faces have mild natural asymmetry before any injection is performed.

Horizontal Forehead Lines

Horizontal forehead lines form when the frontalis muscle raises the eyebrows. Some people make this movement frequently when speaking, reading, applying makeup, or trying to open the eye area more fully. Over time, the repeated folding can leave lines across the forehead even when the eyebrows are no longer raised.

Forehead treatment must be planned in relation to the frown muscles because the frontalis helps support brow position. Relaxing it too much may make the brows feel heavy or alter their shape, especially in someone who already has low brows or excess upper eyelid skin. Current prescribing guidance recommends considering forehead lines together with the glabellar area to reduce the risk of unwanted brow lowering.

Crow’s Feet

Crow’s feet extend from the outer corners of the eyes and become more visible with smiling or squinting. They are created largely by the orbicularis oculi muscle, which surrounds the eye and helps close the eyelids. Xeomin may temporarily soften moderate to severe lateral canthal lines while allowing the treatment plan to account for how much movement you want to preserve.

Some clients appreciate the character that appears around their eyes when they smile and do not want it erased. In those cases, the plan may focus on reducing the strongest creases rather than attempting to remove all visible movement. The most flattering result often comes from respecting the expression rather than overpowering it.

Dynamic Lines Versus Lines at Rest

Xeomin works most directly on lines caused by muscle contraction. If a line appears when you frown but disappears when you relax, reducing that movement may create a noticeable improvement. If the crease remains deeply etched into the skin at rest, Xeomin may soften the repeated folding but may not erase the existing surface change completely.

Static lines are influenced by more than muscle movement. Collagen loss, sun exposure, skin thickness, hydration, and natural aging also affect how deeply a line remains visible. A client with early movement-related lines may respond differently than someone who has had a deep furrow for many years.

In some situations, Dr. Fisher may discuss skincare, microneedling, a professional peel, filler, or another treatment alongside Xeomin. Each option addresses a different part of the concern. The purpose of a combined plan is not to add as many procedures as possible, but to avoid expecting one treatment to solve a problem created by several factors.

Will Xeomin Make My Face Look Frozen?

A frozen appearance usually comes from how a neurotoxin is planned and placed, not simply from the fact that it was used. Dose, injection location, muscle strength, brow position, and personal preference all affect the result. A client who wants strong smoothing for a major event may prefer a different degree of movement than someone receiving treatment for the first time.

Dr. Fisher asks how you want your face to feel as well as how you want it to look. Some clients want their forehead to remain expressive but less lined, while others want stronger control of a deep frown pattern. By watching you smile, squint, frown, and raise your eyebrows, she can build a plan around the way your own face moves.

Clients often tell us that their greatest concern is not looking “done.” They want friends to notice that they seem rested, not immediately identify that they received an injectable. That preference fits the Women’s LifeCenter approach, which emphasizes education, individual choice, and helping each woman reach a place that feels right for her rather than following one aesthetic standard.

Am I a Candidate for Xeomin?

Xeomin may be appropriate for adults who want temporary improvement in moderate to severe upper facial lines and who understand what a neuromodulator can realistically accomplish. A suitable candidate should be willing to discuss medical conditions, medications, previous botulinum toxin treatments, facial surgery, and any past side effects. Treatment should be chosen because the lines genuinely bother you, not because you feel pressured to remove normal signs of expression.

You may be ready to discuss Xeomin when:

  • Frown lines make you look tense or upset
  • Forehead lines remain visible when your face relaxes
  • Crow’s feet have become more prominent
  • You want a temporary, non-surgical treatment
  • You understand that maintenance will be required
  • You want personalized improvement rather than complete immobility
  • You can follow preparation and aftercare instructions
  • You have realistic expectations about the degree of smoothing

Xeomin should not be used in someone with an infection at the proposed injection site or a known allergy to Xeomin, its ingredients, or another botulinum toxin preparation. Dr. Fisher also needs to know about neuromuscular disorders, breathing or swallowing problems, bleeding concerns, drooping eyelids, prior facial surgery, pregnancy, breastfeeding, and all prescription medicines, over-the-counter products, vitamins, and supplements.

Tell Dr. Fisher when you last received any botulinum toxin product, even when it was another brand or was injected at a different office. Certain injectable antibiotics, muscle relaxants, allergy or cold medicines, sleep medicines, and other products can affect treatment planning or risk. Never stop a prescribed medication without speaking with the clinician who manages it.

Your Consultation With Dr. Elysa Fisher

The consultation begins by identifying what you see, when you see it, and how much change you want. Dr. Fisher will examine the upper face at rest and during expression, looking at eyebrow height, eyelid position, forehead length, muscle strength, asymmetry, and the depth of each line. This matters because two clients with similar forehead wrinkles may rely on their muscles in very different ways.

For example, one client may have strong frown lines but naturally high eyebrows, while another may have a lower brow and use the forehead constantly to keep the eye area feeling open. Treating both clients with the same pattern could produce very different results. Dr. Fisher adjusts the plan according to the structure and movement she sees rather than using an identical injection map for every face.

Dr. Fisher completed her undergraduate, graduate, and postgraduate education at Northwestern University and has been board-certified in plastic surgery since 1992. She describes her role as being accessible and serving as a source of information, whether a client is considering a simple in-office procedure or surgery. That broader experience allows her to explain when Xeomin makes sense, when another injectable may be more appropriate, and when the concern is better addressed through skincare or surgery.

What Happens During Xeomin Treatment?

Xeomin treatment is performed in the office and is typically brief. The manufacturer describes an average treatment time of approximately 10 to 15 minutes, although the full appointment may take longer when consultation, photography, facial assessment, and preparation are included. Your skin will be cleansed, and Dr. Fisher will identify the muscles and injection points based on your treatment plan.

The medication is introduced through a fine needle into selected muscles. Most clients describe the injections as quick pinches, although sensitivity varies by treatment area and individual. The number of injection points depends on which areas are being treated and how your muscles function.

The change is not immediate. According to the manufacturer, clients may begin noticing results within approximately four to seven days, although the response continues to develop after that point. The official before-and-after photography is commonly assessed around 14 days, which is why it is usually better to allow the treatment to settle before deciding whether an adjustment is needed.

Xeomin Results and Maintenance

As Xeomin begins working, the treated muscles contract less strongly. Frown lines may look softer, the forehead may appear smoother, and crow’s feet may be less noticeable during smiling or squinting. The result should still fit your face, and individual outcomes vary according to anatomy, muscle activity, dose, placement, age, and skin quality.

Results typically last about three to four months, although some clients notice movement returning sooner or later. The face does not suddenly change back on one specific day. Instead, muscle activity gradually returns, and expression lines become more visible again over time.

The current prescribing information states that aesthetic retreatment should occur no more frequently than every three months. Scheduling should be based on your response rather than automatically repeating treatment before it has worn off. Dr. Fisher can help you establish a maintenance schedule that preserves the result without treating more often than recommended.

Xeomin Aftercare and Downtime

Most clients return to many normal activities after their appointment, but temporary redness, tenderness, swelling, or small marks may appear at the injection sites. Injection-site bruising is the most commonly reported adverse reaction for upper facial line treatment in the current prescribing information. Because bruising cannot be predicted perfectly, schedule treatment with enough time before photographs, travel, weddings, or other important events.

You will receive instructions based on your treatment and health history. General manufacturer guidance includes avoiding strenuous exercise for the first 24 hours, avoiding rubbing or massaging the treated areas, limiting alcohol for at least one day to reduce bruising, and remaining upright for the first several hours. Follow Dr. Fisher’s instructions when they differ from general advice because they will be tailored to your procedure.

Do not press repeatedly on the treated muscles to see whether the Xeomin is working. The effect takes time to develop, and frequent checking can make normal early differences seem more concerning than they are. Contact the office when you have unexpected symptoms, questions about aftercare, or concerns about the developing result.

Risks and Important Safety Information

Xeomin is a prescription medication and can cause side effects. Local concerns may include bruising, tenderness, swelling, asymmetry, brow or eyelid changes, and an outcome that feels stronger or weaker than expected. Some effects improve as the medication wears off, but that process takes time because Xeomin cannot simply be removed after it has affected the muscle.

Xeomin carries a boxed warning because the effects of botulinum toxin can, in rare situations, spread beyond the injection area. This may cause generalized weakness, double or blurred vision, drooping eyelids, voice changes, difficulty speaking, loss of bladder control, or problems swallowing or breathing. These symptoms may occur hours to weeks after treatment and require prompt medical attention, particularly when speaking, swallowing, or breathing becomes difficult.

Allergic reactions are also possible. Symptoms can include itching, rash, redness, swelling, wheezing, dizziness, or faintness. Seek appropriate medical help immediately for breathing difficulty, severe weakness, significant visual changes, swallowing problems, or symptoms of a serious allergic reaction.

Xeomin Versus Botox

Xeomin and Botox are both prescription botulinum toxin type A products that temporarily reduce selected muscle activity. However, their formulations, approved uses, preparation, and biological units are specific to each product. The units cannot be directly compared or converted, so a previous Botox dose should not be treated as an automatic Xeomin dose.

The practical question is not which brand is universally better. It is which product and treatment plan fit your history, anatomy, response, and goals. A client who has used another neuromodulator successfully may decide to continue it, while someone else may be interested in Xeomin’s double-filtered formulation.

Dr. Fisher can explain the differences without assuming that a popular product is automatically the correct choice. She will also want to know when and where you last received any neurotoxin because the effects of different products can overlap. A safe recommendation depends on the full treatment history, not just the brand name remembered from the most recent appointment.

Why Choose Dr. Elysa Fisher for Xeomin in Wilmette?

Injectable treatment may take only a few minutes, but the judgment behind it comes from understanding facial anatomy, aging, muscle balance, and proportion. Dr. Fisher brings the perspective of a board-certified plastic surgeon who provides both surgical and non-surgical care. She can evaluate whether a line is primarily caused by muscle movement, skin change, volume loss, brow position, or a combination of factors.

Her approach also reflects the reason Women’s LifeCenter was created. The practice is intended to give women a supportive environment where they can understand their choices through different stages of life. Clients are not expected to arrive knowing which treatment they need, and they are not reduced to a list of lines that must be corrected.

Clients frequently describe Dr. Fisher as compassionate, detailed, accessible, and willing to answer their questions. They also describe the office as calm, welcoming, and comfortable, which matters when someone is nervous about receiving an injectable for the first time. Those experiences support a treatment philosophy built around trust and personal attention rather than speed or pressure.
